The COSH Director of Health Facilities has told news organizations that a state investigation into the patient’s death has resulted in findings that indicate a need for improvement in operations, staffing and staff training.  It is thought that the staff’s inadequate reaction to the unresponsive woman who later died was the cause of her death but an autopsy report is still outstanding.
